Public Health wraps up village outreach for 2025 with events in Agana Heights and Talo’fo’fo

The Department of Public Health and Social Services is wrapping up its Village Outreach program for the year with two community events in Agana Heights and Talo’fo’fo.
Residents are invited to the Agana Heights Outreach on Friday, Nov. 14 from 10 a.m. to 2 p.m. at the Agana Heights Gym, and the final outreach of the year in Talo’fo’fo on Friday, Nov. 21 from 10 a.m. to 1 p.m. at the Jeremy Newby Community Youth Center.
Families can take advantage of free services including immunizations for children, health education on diabetes and hypertension, and assistance with programs like Medicaid, WIC, and SNAP.
What to bring for Immunizations:
- Child’s birth certificate
- Child’s current shot record
- Children under 18 must be accompanied by a parent. If accompanied by another adult, legal documentation or written authorization from the parent is required.
- Parent, guardian, or authorized adult’s valid photo ID
DPHSS Services Available:
- Immunizations are for children 2 months to 18 years old who are enrolled with Medicaid, MIP or has No Insurance.
- DTaP, IPV, Hep B, Hep A, Hib, Prevnar, MMR, Tdap, Td, MCV, HPV, Annual Flu (While supplies last)
- TB (PPD) skin tests will not be offered, it not a vaccine
- Education on Diabetes, Tobacco, and Hypertension Program Eligibility Assessment on the following:
- Breast and Cervical Cancer Screening Program
- Women, Infants and Children (WIC) Program
- Information and General Assistance on the following:
- Vital Statistics (Marriage, Birth, and Death Certificates)
- Child Care Assistance Program, Foster Parent Information, Questions and Answers about Child Protective Services
- Mental Health & Primary Care Services offered at the Community Health Centers
- Public Welfare Applications (SNAP, MIP, Medicaid, Cash Assistance)
- Aging Programs - Title III Home and Community Based Services
