The Department of Public Health and Social Services Maternal Child Health Services through the Bureau of Family Health and Nursing Services and DPHSS Immunization program continues to host child immunization clinics:

Child immunization clinics are for children 4 years – 18 years of age, and limited to those with Medicaid, MIP, or no insurance. Excluding the varicella (chickenpox) vaccine, all necessary routine child immunizations will be administered. The child’s most current shot record must be provided. TB (PPD) skin tests will not be administered.

DPHSS Southern Region Community Health Center (SRCHC), Inalåhan

Call (671) 828-7604/5/7518 to schedule an appointment. Last appointment is 30 minutes before closing. Priority is given to patients with appointments. Walk-ins welcome. Curbside vaccination available for people with disabilities.

Homebound COVID-19 vaccination continues to be available Monday through Friday from 8 a.m. to 5 p.m. Eligible residents may call 311 and select option 1 or option 2, or contact their village mayor to schedule homebound COVID-19 vaccination.

MINOR CHILD REQUIRED DOCUMENTS

Requirements for children less than 18 years old to receive routine child immunizations is as follows:

Parent/legal guardian/authorized adult must provide minor’s birth certificate.
Parent/legal guardian/authorized adult must have a government-issued photo identification.
Minor must be accompanied by a parent, legal guardian or an authorized adult.
Guardians must present legal guardianship or power-of-attorney documents. Authorized adult must present signed written authorization from parent.

ROUTINE IMMUNIZATIONS

Additionally, DPHSS Community Health Centers offer immunization services. Priority is given to patients with appointments. Please note that walk-ins are based on availability and do not guarantee you will be seen.

DPHSS NRCHC: Monday – Friday from 8 a.m. to 6 p.m. and Saturday 8 a.m. to 5 p.m.

While DPHSS continues to make every reasonable effort for flexible and convenient routine immunization and COVID-19 vaccination, the community is advised that schedules for vaccination and child immunization are subject to change.
