Guam - GM Logistics has been cited for safety violations following the death of a forklift operator last November. The Occupational Safety and Health Administration conducted an inspection after 48-year-old Paul Blas died after he was crushed under a forklift that had toppled over. 

GM Logistics was cited for allowing an employee to operate the defective forklift without using a seat belt.  According to news files, Blas was trying to move a 40'-long bus from a privately owned lot in Harmon to the Formosa Recycling Facility. OSHA determined the death was preventable and issued proposed penalties of nearly $20,000.  The company has 15 days to comply with the citations or contest the findings.
