SNAP households may be able to replace some foods purchased with their SNAP EBT card and lost due to Typhoon Mawar. 

The Department of Public Health and Social Services is launching a Food Loss Program, which replaces food lost in May. The replacement will be considered using information presented in signed affidavits by SNAP family head-of-households and submitted to DPHSS. 

The last day to submit affidavits is June 21, 2023.   

The following steps or instructions are listed below: 

Step 1. Obtain the Affidavit. 

 

The affidavit forms are available online at https://dphss.guam.gov/division-of-public-welfare/ and at the following locations, Monday to Friday from 8 am to 5 pm, excluding Government of Guam holidays:  

 

  • Ran Care building in Tamuning 
  • Northern Community Health Center in Dededo 
  • Southern Community Health Center in Inalahan 
  • Disaster Recovery Center (DRC) at the Guam Community College, Building E, Sesame Street, Mangilao 

 

Step 2. Fill out the Affidavit. 

 

The affidavit form will ask for the following: your nameaddress, date of birth, and SNAP case number. Fill out the estimated dollar value of food lost as a result of the typhoon and check all the boxes which state the applicable reasons your food was lost. Should there be reasons other than those listed in the affidavit form, check “Other” and write the reasons or causes of the food loss in the spaces provided. 

 

Step 3. Sign the Affidavit.  

 

The head-of-household must sign the affidavit. The affidavit does not need to be notarized. 

 

Step 4. Submit the Affidavit. 

 

The affidavits must be submitted to DPHSS, and can be submitted through the following methods: 

 

  • Drop off the signed affidavit to any of the locations listed in step 1. 
  • Individuals can send a digital photocopy of the signed affidavit through Whatsapp at (671) 727-2462 or 727-1351, these numbers will only accept Whatsapp messages for the Food Loss Program. 
  • Individuals can also send a digital photocopy of the signed affidavit through email at [email protected] with the subject line being “Food Loss Affidavit.”
